Add encounter timestamps and consent statement option in notes

Requested the ability to automatically include a timestamp in the note that shows when the encounter started, when it ended, and the total duration. This is important for billing purposes, and for the need to document session length.

In the top right hand corner of the window, the duration of the appointment is logged; I am in Ontario and some of our billing codes are time-based and we have to document start and stop times. A useful feature would be to have the start and stop times documented beside the appointment duration. Alternatively embed the start and stop times and appointment duration at the bottom of the note.

Thank you for reaching out! Here's how users can view that information on a Heidi session (with an accompanying screenshot):
- Encounter Start: Displayed on the top left of the session.
- Total Duration: Shown with a clock icon in the top right corner of each session
However, we don't currently display the session end timestamp. Does the information above meet your billing needs, or would the end timestamp be helpful to see?
