As a psychiatrist, I need to create a "Treatment Plan" document that is outside of the normal visit notes. It documents the clinical problems that are being addressed, what the goals of treatment are and how they are measured (such as anxiety score is none or mild using rating scale GAD-7, or based on patient report, etc). Subsequent treatment plans will make an assessment of if the goal was achieved, if the goal is the same or has changed, any new goals, and any changes to the treatments that are being used to further that goal. Treatments include medications, diet, exercise, getting daylight, earthing, sleep hygiene, CPAP use, following up with specialists, etc. It states when the next treatment plan review will be (typically 3-6 months, but may be longer or shorter based on patient status.). I would like to be able to upload the patient notes for the time since the last treatment plan update and auto-generate a treatment plan. Ideally, it would fit on one page or at most 2 pages. Excessively long treatment plans are typically not all that useful.
