Tim reports that the default note template does not populate automatically and the generated output no longer reflects the transcript. He currently needs to manually refresh with Control+O for every interaction to update the note. This causes an estimated delay of at least 20 minutes per shift. He notes that requesting an updated output based on the transcript sometimes provides a reasonable note, but the auto-sync functionality that previously existed is no longer working reliably.
