Thanks for the suggestion, Hayley Dutton — having the consult date clearly visible at the top of each note would definitely make it easier to scan and file notes throughout the day.

A couple quick questions so we can share the right detail with the team:

1) Do you want the date only, or date + time as well?

2) Should it reflect the consult date (appointment date) or the note creation date?

3) Where exactly should it appear (e.g., first line above the header, or inside the header area)?
