Hallucinating too often with medication and differential diagnosis

I'm getting very concerned just how often Heidi is miss hearing and typing antidepressants, It seems to default to citalopram or venlafaxine far too often. Less so with antibiotics while other meds seem to always be right,
I'm also getting increasingly worried by it adding differential diagnosis that I have not explicitly discussed with the patient. Its now taking me longer to edit it than it does just to type the consultation.

Thanks for flagging this, Fingal O'Reilly — totally hear you, that’s really concerning (especially around meds and adding differentials you didn’t discuss). We’re going to dig into what’s causing the antidepressant mix-ups (e.g. defaulting to citalopram/venlafaxine) and the extra differential diagnoses, and I’ll come back to you once we’ve looked into it.
