I would like my assessment/plan to be written in complete sentences, not point form

I have tried to instruct in my consultation template to do the assessment/plan in full sentences, not point form. However, it still comes out in point form.

- Have you noticed if this issue occurs with specific types of assessments/plans or is it consistent across all?
- Is there a particular section of the assessment/plan where using complete sentences is most critical for your workflow?

1) Instead of something like this:
- Discussed options for delivery
- Reviewed risks of surgery, including bleeding and infection
I would like it to be like this:
We reviewed options for delivery and risks of surgery, including bleeding and infection.
2) This occurs with all the assessment/plans, despite my instructions requesting full sentences.
3) The assessment/plan is the only section where I would always like complete sentences.
