eg a drug has been tried rather than trialled, stopped rather than discontinued, 'chest clear' rather than chest auscultation normal.
I'd also like to be able to make changes to the transcript which Heidi learns from and starts to incorporate.

Hi Ian, thanks for the feedback! The most reliable way to make this work is by adding this instruction to your template via our template editor to each template where you'd like this rule to apply.
As mentioned we also have a Personalisation feature which learns as you edit and we're currently working on making improvements to this feature.

The personalization option works relatively well with this sort of thing. Within the document, change the text to what you want it to look like, and the personalisation may pick it up and learn it as a rule. If you spend some time doing this over time, it does seem to shape the language to something that fits more closely to one's natural speaking voice.
