Link Patients Across Providers

For Teams-enabled accounts, it would be helpful if a patient's profile spanned all practitioners on the account. If one patient is visited by multiple practitioners on the same account, it would be helpful if the patient's profile could be linked across accounts.

Hi, as team we see the same patients on rotational basis. So we should be able to find the existent patient profiles in the team records and link them to any new attending team Dr or link all records to automatically since its the same patient. This will allow for continuous record keeping and better service provision to the pts. Currently we have to re-create the profile multiple times for each team Dr and copy/paste previous records to "Context". Takes time and is prone to errors. Also none of this is doable at the bedside where we need it the most. Hence, we are lacking vital information at the bedside. We are not GPs, hardly ever seating in the office.

Dear Liz. Thanks again for your response. No, we will not restrict access to any members of the team. All team members should have the access to all records. Patients are moving between treatment centres, hospitals, rooms etc. Doctors rotate as well. So in case of emergency or high work load volume such restrictions are not helpful. Kind Regards

Hi Vakhtang Rekhviashvili, thank you so much for this feedback! This is something our team already has planned in our roadmap to support. We want to make it easier for a whole team to work on the same patient profiles and keep that context as a patient moves from one practitioner to another.
A question to help us build this out well: are there ever scenarios where you would want to restrict who has visibility to a patient profile? Or would you typically share patient profiles across your team?

If a practice has upgraded to "Practice," all patients should exist in a single database.

It is too much work to go through and find last visits in other providers databases. It makes upgrading not as valuable and difficult to recommend.

I always assumed that with practice you can click the left sidebar, then click "me" to change the user. Or is that not the case?

Hi both -- thanks for that feedback! Shared patient profiles is something we're currently reviewing to enable better collaboration on the same patient.
One question for you: are there ever scenarios where you would want to restrict who has visibility to a patient profile? Or would you typically share patient profiles across your team?

Liz from Heidi Potentially if there are different departments operating within one organization or Team account. For instance, our practice sees patients at two distinct types of facilities so visibility of patients being seen at one facility setting might create a "noisy" environment for providers that operate out of the second facility type.
Even still, I think the continuity of care creates a benefit that outweighs the additional noise of "irrelevant" patients and even helps in the event that a patient does visit providers in distinct departments.

Liz from Heidi- HI Liz, I think the simplest would be share all. No doubt there might be some who'd want to toggle the occasional patient "off" from sharing mode

Liz from Heidi - In my clinic with 2 providers I do not have a scenario where I would need to restrict visibility.
Maybe a toggle option for choosing which providers link to share notes would allow for smaller and larger clinics to customize.
Here's a clinical example. My specialty is orthopedics. My PA sees a new patient with a fracture and then the patient follows up with me 2 weeks later to check progress. I have to find a way to copy and paste all of his documentation into the context so I do not have to repeat history, diagnosis, etc. It is not easy to change provider profiles, search for patient, copy note, switch profiles, find patient, copy to context.

This would be extremely helpful to have the context of other providers notes in generating my note. For my clinic, my PA and I alternate visits with patients and we have to copy context from the EHR since the visits are not linked between us.
