our small team of clinical pharmacists assist the GPs by completing a medicine reconciliation when patients transit care between primary and secondary care. We read the letters that come into the inbox, update any medicine changes and contact the patient. I would like to see if Heidi can read the letter, compare against an existing list of medications and notify the clinical pharmacist for any changes. then using a new template, document the reason the letter came in (the diagnosis) , relevant issues or plan from the letter, changes to medicines and any discrepancies. then the clinical pharmacist can take it from there. is that possible
