Previsit questionnaire that automatically feeds info into the note

I would like for pt to receive a previsit questionnaire prior to the appointment that automatically links to their visit and feeds information into the note. It would be great if it also imported structured data into the EHR.

Thanks for sharing this — a pre‑visit questionnaire that links to the encounter and pre-fills the note (and ideally pushes structured fields into the EHR) would be a big workflow win. I’ll pass this along to the team.
A couple quick questions so we understand the ideal flow:
1) What kind of questionnaire data do you want captured (e.g., HPI prompts, ROS, PMH/meds/allergies, screening tools like PHQ-9/GAD-7), and which parts should land in the note vs. as discrete EHR fields?
2) Which EHR are you using, and are there specific structured fields you’d want populated (problem list, meds, allergies, vitals, questionnaires, etc.)?
3) How do you want patients to receive/complete it (SMS/email link, patient portal, in-clinic on a tablet), and how soon before the appointment?
If you have an example of your current pre-visit form (or a screenshot/template), that’d help us map it to the right note sections and integration approach.

1 - all of these would be helpful. I would want different information on a first visit as compared to a return visit. And any objective data - vitals, screening tool numbers, etc - should go into structured data.
2 - Elation is our current EHR
- SMS/email would be great. Ideally offering a week prior to visit for new patients and 48 hours prior to visit for return visit. It would also help to have reminders.
