I'd like my notes as a therapist to be less detailed and more of a basic summary of the themes we discussed, the strategies and interventions used, and the client's receptiveness to therapy that is appropriate for an insurance company's needs more so than an actual play by play. Even the "less detailed" version gives lots of personal details and this is not what insurance companies are looking for.
