Currently, when sessions are linked to a patient profile, notes from previous sessions carry across, but uploaded documents (intake forms, consent forms, prior records) do not. As a clinician, this means I have to re-upload or re-paste context each time I start a new chat for the same patient, which limits the value of linking sessions under one profile.
Suggested enhancement:
Allow attachments and uploaded documents associated with a patient's prior sessions to be accessible in subsequent sessions for the same patient profile. This would reduce duplicated input and improve continuity of care across visits.

It sounds like the main gap is that linking sessions to the same patient profile carries over note context, but not the documents that are often just as important for ongoing care. Having to repeatedly upload intake forms, consent forms, or prior records adds unnecessary work and makes the patient profile feel less continuous than it should.
I’ve passed this on as a request for uploaded documents and attachments to remain accessible across sessions linked to the same patient profile. One thing that would be helpful to understand: would you want all prior patient documents to be available automatically, or would you prefer to choose which documents carry forward into a new session?
