Title: Issues with pushing data and context tab in PQ widget

Description:

User reports that in the PracticeQ widget:

Structured data doesn’t update or push more than once.

Context tab data doesn’t carry over into notes if not clicked before ending a session.

Previously, they could generate a new note from the widget, but this feature has stopped working.
