Subjective info is frequently put into the Objective section, even when very clearly subjective. For example, a recent note I generated put a list of symptoms in the Objective section.

I have this issue too but it has only become an issue in the last few weeks or so. Not sure what changes have been made. It will frequently miss assessment as a heading altogether, even when assessment info is in the note. It will be in subjective instead.
