Hello,
By default the output always refers to “patients” however in psychotherapy and counselling in the UK we mostly refer to people as clients - it would be good if there was a way to define this descriptor in the settings for existing standard templates.

I would like something like dragon. For example if you says disc annular tear. It comes about with a paragraph of information. Also example, I used to say "discharge 1" , " discharge 2" and "discharge 3" it would generate different sentences. This speeds up the process.

It would be useful to define and use tools to generate documentation e.g. text messages that take in parameters (instead of a transcript, but based on the existing template structure) but that can be linked to a patient. Tools could be single/multiline textboxes, ranges, numerical, booleans etc.
For example, a tool may be re: information about B12 deficiency with a link - creating a text message. The parameters would be "Topic" and "Link" (possibly a "summary of information" field as well).
This would then generate the text:
Dear [patient name],
Please see the following information about B12 deficiency:
link...
B12 is a... etc

Being able to insert any specific dot phrase. Use a symbol to insert like on other EMRs (such as a procedure note/signature disclosure/discussed consent, etc which you want in exact verbiage like a dot phrase, not what the AI wants it to say)

Have a few examples:
1) Being able to set a global rule for measurement/anthropometry would be helpful - e.g, if I set anthropometry measurements to 'metric > kg, cm', and a client tells me that they are "5 foot 9 inches and weigh 11 stone", Heidi would automatically convert that to:
- Height: 175 cm
- Weight: 69.8 kg
in the relevant section.
2) Similarly, having a setting to "always format specific dates", e.g., if a client says "my blood pressure was "110/70 this morning, much better than the 130/96 last Tuesday", Heidi would check the current date how "last Tuesday" relates to this and use the appropriate dates, in this case:
- Blood Pressure: 110/70 mmHg (02/09/2024), 130/96 mmHg (27/08/2024)
3) Being able to set consistent formatting for blood tests/medications - currently if a client tells me that their HbA1C "was 6.7 last Tuesday, much better than the 7.8 I had last year on December 12th last year, or when it was 7.2 earlier this year on March 15th" (i.e, no specific date provided like "last tuesday", "last year", and out-of-chronological order), Heidi might currently write this as
- Client reports HbA1C was 6.7 last Tuesday, previously 7.8 (12/12/2023), 7.2 (15/03/2024)
Whereas I'd love to be able to set something like "Organise investigations > reverse chronologically" and "Use new line for each investigation" or something similar so that the above would instead be output in the note as:
- HbA1C:
- 6.7 (27/08/2024)
- 7.2 (15/03/2024)
- 7.8 (12/12/2023)
4) Echoing others' sentiments: setting a global preference for "client" vs "patient" vs. "first-name basis", etc.
5) Echoing others' sentiments: being able to select the formatting of dates as DD/MM/YYYY or MM/DD/YYYY or YYYY/MM/DD, etc.
6) Echoing others' sentiments: Re: when you fix a spelling error (e.g. name, medication, etc.) that Heidi has made consistently throughout the note, a macro that automatically applies the change to all further instances of the misspelled word.

Continuing from my previous comment:
7) Being able to define acronyms (e.g. T2DM instead of "type II diabetes mellitus", GORD instead of 'gastro-oesophageal reflux disease", etc.) and whether we would like to use the acronyms instead of the full terminology.
This would also allow less common or discipline-specific acronyms (like ID for intellectual disability or ND for neurodivergence)
7.b) Related; A macro to use the full term in a correspondence letter/referral/etc. only in the first instance and use the acronym in subsequent references; e.g. "Thank you for your referral to assist Mr. Smith with his type II diabetes mellitus (T2DM). Mr. Smith is currently managing his T2DM with lifestyle changes..." etc.
8) Being able to set a global setting for preferred terminology; some examples:
- "ASD" vs. "autism spectrum disorder" vs. "autism" vs. "autism spectrum differences" vs. "neurodivergence"
- "diet" vs. "eating pattern" vs. "routine oral intake"
- "physical disability" vs. "physical impairment" vs. "impaired mobility" vs. "reduced mobility" (e.g. in reference to someone that mobilises with a wheelchair)
9) Similarly, a setting for "practitioners own shorthand" that would only affect case notes but not other outputs; e.g. in my own notes I'll always use "reqs" for 'requirements', "CHO" for carbohydrate, "EtOH" for alcohol, "Hx" for 'history', etc., but wouldn't necessarily want these output in letters, etc.

I will have more but some medical specialties want the word "client" vs patient. But if I type in the patients name (my staff preloads all patients before I see them), then the name should be the default. For those that want client, it seems to be mostly like psychology, then just build that into the template. Done. Dates should follow the region rules. For US just build the way we do our dates vs the EU into the template. Done.
I am thinking of macros differently. Less button pushes per se. I want a default patient summary each visit. Some patients in the US want it but some don't. If I do a visit like what Freed AI does it creates a patient summary automatically. This gives me a choice and no button pushes. Most of my corrections is lately the AI not putting in the wrong name of the drug. Suggestion of right click is great. But the AI needs to hold the correction. If I correct a word THEN restate something or add more information into the note the AI doesn't hold the correction but I will admit I think I saw it last week actually hold the correction once.
There used to be a button for less quotes. Then it was taken away. Now I have to type or speak in, remove the quotes. Quotes is not consistent. Sometimes it's just fantastic and minimalistic. Then the next patient it like quotes what was exactly said each line. Using right brain and detailed. Left brain leaves out too much important detail in the note if you compare side to side.
I would still like "voice" macros. I want a back to work /school form. I should be able to just ask for it. BTW - Freed AI can do this with just about any report or letter. So the technology is there to accomplish this. Just ask for a referral note to Dr Johnson on the current patient. Done.

In psych, I have a pretty standard outline of a mental status examination and particular wording that I like for consistency. Having dot phrases or even organization macros could help with readability as sometimes when I dictate, the AI will write it out in my spoken phase (incomplete sentence) as a direct quote or that it will pick and choose what to put into some narrative inclusive sentence. Sometimes it will pick up when I reflect observations in session though this is inconsistent.
Where to organize psychotherapy interventions (and their response) within the SOAP format could be helpful as it appears to be inconsistent with the AI deciding where (or if) they will go.
AI appears to be getting better with keeping up with when I put context in for pronouns though sometimes will come up with alternative spellings or just drop the pt's name in the notes and go genderless "patient"

There are a couple of transcription that all voice recognition systems / seem to make consistently. In my specialty of rheumatology (as I’m sure lots of others) a misinterpretation of a dose eg 50mg of methotrexate vs 15mg can be fatal and I want to put in a rule that means it’s NEVER 50mg of methotrexate (there are lots of other examples I could give). At the moment it will correct one note but not all of them automatically.
