A template for biopsychosocial formulation for psychologists and psychotherapists. This would include the four P's (predisposing (genetic, socioeconomic, psychological from childhood), precipitating (medical, current social circumstances, current mental health), perpetuating (ongoing biological, social and psychological factors), protective (health, family and/or social support, psychological protectors). These should be split into a table with the P's down the left side and biological, socioeconomic, and psychological along the top to create 12 boxes to be completed with information from the client/patient interview.

Hi Zoe, I have had some success with an assessment template including formulation. I copied a comprehensive assessment template (Word document) including multiple considerations history, four P's etc. into a new Heidi template. I then followed the template tips using " " and [ ]. When used for a session the result is quite impressive - the information is categorised and the four P's are completed from the information provided. It is not presented in table form but obviously a table could be made from the resulting notes. I'm happy to share the template if needed.
